Location Requirement
Candidates may reside anywhere on the East Coast (EST time zone).
Must hold an active, unrestricted New York RN license.
Key Responsibilities
Conduct telephonic assessments using comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS).
Perform in-person assessments for non-MLTSS members to evaluate medical needs and determine appropriate service referrals, including:
Adult Medical Day Care
Pediatric Medical Day Care
Personal Care Assistant (PCA) Services
Nursing Facility Custodial Care
Personal Preference Program (PPP)
MLTSS Enrollment
Work with diverse populations, including pediatric and medically complex members.
Coordinate care with members, authorized representatives, primary care providers, and interdisciplinary care teams.
Schedule and participate in interdisciplinary care meetings while advocating for members to ensure safe transitions and appropriate services.
Develop individualized care plans and authorize services within MLTSS/FIDE program benefits in a cost-effective manner.
Accurately document assessments, care plans, and member interactions in the electronic health record.
Utilize strong critical thinking and problem-solving skills in daily case management activities.
Mentor new staff after achieving role proficiency.
Required Qualifications
Active, unrestricted Registered Nurse (RN) license in the state of New York.
Minimum 3 years of clinical RN experience in one or more of the following:
Hospital
Home Health
Ambulatory Care
Fluent in English and Spanish (reading, writing, and speaking).
Associate Degree in Nursing (minimum).
Preferred Qualifications
Case Management experience
Discharge Planning experience
Managed Care experience
Crisis Intervention experience
Certified Case Manager (CCM) certification preferred
