Fill Annexure-A in block letters. State date of birth, date of entry into service, date of retirement, qualifications, and how you meet each essential condition. Attach attested copies of ACRs/APARs for the last five years and the vigilance clearance certificate. Ask your cadre controlling authority to forward the set while your services can still be spared immediately.
